MODELLING APPLICATION FORM - TEST SHOOT (part 1)

PLEASE READ THIS BEFORE COMPLETING APPLICATION FORM


1.	Please make sure you have read the WANTED page first as our needs change from time to time. Free shoots or test shoots are only done with those who suit our needs as specified on the wanted page.

2.	At this time we are only in a position to promote female models and so will only process applications from female models!

3.	After completing the first page of the application form, it will be necessary to send a photo of yourself for our evaluation. While we do not wish to offend, not everyone is suitable for modeling and we reserve the right to reject applications from those we consider unsuitable. We do not make judgments based on height, weight or age. A follow up phone call from the applicant is required to verify that the application is genuine.

4.	Our test shoots are free, but we cannot waste our time on models who do not show for their appointments or who arrive late without advance notice of at least 24 hours.

5.	All models must be at least eighteen years of age.

6.	All fields in the form require an entry.

7.	All models must be based in South Florida. (or travelling to or able to travel to FL at their own expense)

You may otherwise find yourself halfway through a Lingerie shoot and rejecting a clients product which is not only embarrassing but very inconvenient for all concerned.
